The ductus venosus is one of the key fetal shunts that play a vital role in fetal circulation. This shunt connects the umbilical vein directly to the inferior vena cava. The primary function of the ductus venosus is to bypass the liver, allowing oxygen-rich blood from the placenta to flow directly to the fetal heart. By skipping the liver, which is not fully functional in the fetus, the ductus venosus ensures that vital nutrients and oxygen reach the developing heart promptly. This efficient pathway supports the rapid growth and development of the fetal organs.

Once the baby is born and the umbilical cord is clamped, the ductus venosus closes and becomes the ligamentum venosum, a non-functional ligament.

The foramen ovale is another crucial element in fetal circulation. It is an opening between the right atrium and the left atrium in the heart. During fetal life, this opening allows blood to bypass the non-functional fetal lungs. Since the lungs are filled with fluid and not yet used for breathing, the foramen ovale enables blood to flow directly from the right atrium to the left atrium, bypassing the pulmonary circuit. This shunt is essential to ensure that oxygenated blood from the placenta can reach the systemic circulation quickly.

After birth, as the newborn starts to breathe air, the pressure in the left atrium increases, leading to the closure of the foramen ovale. It eventually becomes the fossa ovalis, a small indentation in the atrial septum.

The ductus arteriosus is the third significant fetal shunt. It serves to connect the pulmonary artery to the descending aorta. This shunt is crucial for directing blood away from the underdeveloped fetal lungs. Because the lungs are not yet needed for oxygen exchange in the womb, the ductus arteriosus provides a pathway for blood from the right ventricle to bypass the pulmonary circuit and enter the systemic circulation directly. This efficient system helps maintain proper oxygenation of the fetal tissues.

Within a few days after birth, as the newborn begins to breathe and the lungs start functioning, the ductus arteriosus usually closes. It then forms the ligamentum arteriosum, a fibrous remnant of its earlier role in fetal life.
